With COVID-19 just around the corner, there is no such thing as risk-free travel. What the people can do now to protect their health is not just rely on cure but rather rely on prevention by following health protocols imposed in each country you plan to travel to.

In Thailand, there are several requirements that a Thai national returning to Thailand and international travelers arriving in Thailand need to show before being granted entry to the country. Included in the requirements is the document that monitors health upon entering Thailand, which is the Health Declaration Form or also known as the T8 Form.

Health Declaration Form or T8 Form

Health Declaration Form or the T8 Form is a form that is required to be filled in by all travelers entering Thailand. It is part of the initiative by the Thai government or Thai authorities to monitor travelers entering Thailand and to contain the spread of the COVID-19.

What to Expect in a Health Declaration Form or T8 Form

In a Health Declaration Form or T8 Form, travelers will be required to fill in personal details such as names, contact numbers, e-mail addresses, flight numbers, and other necessary information that will be helpful to determining and reaching out to the travelers entering Thailand.

To monitor the health and symptoms of travelers, the Health Declaration Form or T8 Form also requires travelers to check their selves if they have the following symptoms:

  • Diarrhea
  • Vomiting
  • Abdominal Pain
  • Fever
  • Head Ache
  • Sore Throat
  • Rash
  • Jaundice
  • Cough
  • Shortness of Breath
  • Enlarged Nymph Glands or Tender Lumps
  • And other health complications felt by the travelers that need to be specified by themselves.

The personal information you supply will mostly be utilized by Thai health officials for contact tracing if a COVID-19 case occurs on your aircraft or in your immediate proximity. 

Note: After the arrival date, your data will only be kept for 14 days.

All Travelers Entering Thailand Requires Health Declaration or T8 Form

Without regard to nationality or visa status, according to Section 12(7) of the immigration law, anyone who refuses to comply with the new regulation will be restricted from passing through immigration checkpoints and denied entry into Thailand. 

Note: Those who give false or erroneous information will face legal consequences.

Accessing and Obtaining Thailand Health Declaration Form or T8 Form

Travelers can have access to the Thailand Health Declaration Form or T8 Form the following:

  • The Airline (Airlines provide T8 Form)
  • AOT T8 Form App
  • Print one yourself

At all international airports in Thailand, you will be able to utilize the Thailand Health Declaration (T.8 form) when you arrive.

Note: Obtaining, and completing to fill out this form does not guarantee your entry to the Kingdom of Thailand. There are still requirements other than the Health Declaration Form or the T8 Form that travelers, both Thai nationals and non-Thai nationals need to bring or show before departure and upon arrival in Thailand.

Other Requirements other than the Health Declaration Form or T8 Form

As mentioned earlier, the Health Declaration Form or the T8 form is just one of the requirements. Listed below are the other requirements:

For Thai Citizens/Nationals returning to Thailand:

  • Thai Passport
  • Confirmed booked ASQ/SHA+ hotel
  • Confirmed booked flight.
  • Certificate of Entry (COE)
  • Medical or Health certificate issued at least 72 hours before the intended date of departure.
  • Certificate of Vaccination (vaccines should be approved by the World Health Organization or the Ministry of Public Health)
  • Downloaded ThailandPlus Application

For Non-Thai Nationals visiting/entering Thailand:

  • Declaration Form
  • Certificate of Entry (COE)
  • Install ThailandPlus Application
  • Valid Thai Visa or Re-entry permit
  • Valid Passport with at least 6 months validity
  • COVID-19 Health Insurance Certificate
  • Confirmed Booked ASQ Hotel or SHA+ Hotel
  • Medical or Health Certificate stating a negative result from COVID-19. It shall be obtained not more than 72 hours before the intended date of departure. 
  • Vaccine Certificate (vaccines should be approved by the World Health Organization or the Ministry of Public Health)

Travelers Subject for Mandatory Quarantine

Aside from being health monitored by Thai authorities, for further security, protection, and prevention from COVID-19, Thai nationals and non-Thai nationals returning/entering Thailand are required to stay in a secured area for a specific number of days to make sure that they are COVID-19 free.

The number of days they are required to stay depends on their vaccination status:

  • 7 days if the travelers are fully vaccinated
  • 10 days if the travelers are unvaccinated
  • 14 days if the travelers are unvaccinated and are traveling by land/sea

With that in mind, the Thai authorities have given options of the secured areas for travelers to choose from:

  • Hotel Quarantine Program
    For fully vaccinated and unvaccinated travelers who just want to stay in hotel rooms, this program is for you. 

    Upon arrival in Thailand, they will be transferred to their booked ASQ hotel and are required to stay there and are not allowed to go out of their hotel rooms.

  • Sandbox Program
    For fully vaccinated travelers who want to stay in a secure area while having the privilege to roam outside the hotel premises and are allowed to do outside activities, this program is for you.

    Upon arrival in Thailand, they will be transferred to their booked SHA+ hotel and are required to stay there while having the privilege to go outside.

Travelers are only allowed to travel to other Thailand destinations once completed their required number of stays.

Purchase COVID-19 Insurance

With the Health Declaration T8 Form monitoring your health for 14 days upon arrival, it is just right that if contracted with COVID-19 there is health insurance to save you from costs caused by the COVID-19 during your trip and stay in Thailand.

In purchasing your COVID-19 insurance, make sure that it has a minimum medical coverage of THB 3.5 million ($100,000) and make sure that it will cover the following:

  • Medical expenses (including medications, professional care, etc.)
  • Life Insurance (in cases that involves death)
  • Unexpected expenses caused by COVID-19 incidents (like loss of baggage, or missing of scheduled flights)
